From the Editor's Desk

Alhamdolillah the first issue of HeartBeat-The CP newsletter for Health Professionals
is on your screen.

The idea of launching this newsletter along with the Virtual Hospital was conceived by Tahir Bhai of CP. It was strong motivation by him that led to identification of an energetic team of volunteer editors, dedicated contributors and well wishers. We were initially reluctant to bring out a small, imperfect issue but then we decided that at least when we will take a start we will have a lot of you contributing for us.

Existence of Journals and newsletters depends on the contributions from readers. Although many of you have already contacted us and we have accumulated some material for a few proposed sections yet we are grossly deficient in others. We request you to kindly send us your valuable contributions. The management wants all sectors of health sciences, Doctors, Nurses, Biomedical Engineers, Pharmacists, Technologists, Paramedics, Contemporary Medicine (to name a few) to be represented properly.

We are open to suggestions. The newsletter can have any section of your choice. Please send us your feedback to improve the quality and contents of the next issue.

In addition we need to finalize the editorial board. We invite health professionals with experience in medical editing, medical writing or medical journalism to kindly come forward and submit their names and resume. We have a task and a position for everyone. Trust us we will not ask for much time or labor.

With best regards

CP Launches the First Virtual Hospital of the Middle East

Tahir Butt of www.ContactPakistan.com (CP) launched its dream project of the Virtual Hospital for its Saudi Arabian (KSA) and the Gulf region CP members. His mother who actuated the system by pressing the golden key of the laptop launched this. “I am most honored and pleased to announce today that CP membership in KSA has now exceeded the legendry mark of 50,000. And, on this momentous occasion I wish to inform you all that we are now set to launch the CP Virtual Hospital with immediate effect”, he disclosed this in a press conference called for the purpose. According to him, Virtual Hospital has been a long lasting demand from the CP members worldwide and launching of this Virtual Hospital at this time was intended to redeem that age-old demand. Upon a query, he informed that the CP Virtual Hospital would be a hospital with doctors but without a building. Through an online request form any body would be able to put down the questions/queries describing the medical ailments being faced by the incumbent. This request form, once duly filled out, could then be directed to the doctor concerned from amongst the list of the panel of doctors provided. Once the request form has been submitted the sender would then receive, in his/her inbox, the first response from the concerned doctor within 48 hours.

Know the Place
 where WE Work

Hospital is apparently "just another building", made up of same steel, concrete, fiber and glass. However not everyone knows the complexities involved in planning and placing each and every brick of this highly sophisticate structure designed for catering to some of the most dangerous situations faced by humans. HeartBeat expert of hospital engineering will try to highlight these very complex issues in simple language.

Interaction with patients, patients relatives and general public is part of a health professional's life. This is usually coupled with immensely tense decision making moments for patients as well as health professionals. This gives rise to classical scenario for slip of tongue & interesting behaviors, both in health professionals and in patients. When we are free we recall these events to relax our tense nerves and "laugh it out".

HeartBeat Experts have selected a few abstracts from the most recent issues of reputed medical journals with an idea of continuing medical education (CME). This issue includes abstracts on following topics:

  • Vasopressin & epinephrinein cardiopulmonary resuscitation

  • Motesanib diphosphate in progressive differentiated thyroid cancer

  • Laparoscopic repair of inferior vena caval injury

  • Anastomotic leakage after resection for rectal cancer

  • Chronic pilonidal disease treatment

  • Abdominal wall endometriosis

  • Sentinel node mapping in breast cancer patients

  • Insulin Glargine Versus Intermediate Acting Insulin

Precautions In use of MRI Equipment

MRI Equipment is unique in the sense that it is one of the most sensitive equipment in the medical imaging field. Since it uses very powerful magnets many precautions must be taken when setting up an MRI suite. Purchasing the correct MRI equipment is essential for both patient and staff safety. The equipment around the MRI machine must be completely non magnetic otherwise they will be pulled towards the MRI machine at a dangerous speed. There are many negligent MRI accidents every year in facilities because people do not respect the power of these machines.
MRI machines can turn equipment in the room into flying projectiles if they are not MRI tested and approved. There are manufacturers who specialize in making and testing MRI equipment for use in an MRI suite. Theses manufacturers choose to make their products out of non magnetic elements. The most popular of which is stainless steel. As a substitute for metal often times the MRI equipment is made of PVC material, since it is plastic and therefore qualifies as non magnetic.
When it comes to purchasing the actual MRI machine itself you must consider first patient comfort. One of the most difficult things for a patient in an MRI is that they can easily become claustrophobic. Since MRI machines are usually in an enclosed tube. A solution to this is to purchase an open MRI machine in which there is not tube for the patient to feel confined in.
Whether purchasing an open or close MRI machine you need to outfit your room with MRI equipment that is compatible and safe. In order to avoid being negligent in your testing you need to make sure that all magnetic objects are cleared from the range of the magnets.

Common bowel problem linked to chilli pepper pain receptor

People with irritable bowel syndrome have a higher than usual number of chilli pepper pain receptors, according to a new study published. The research could lead to new therapies for the estimated one in five UK adults who have irritable bowel syndrome (IBS), a painful condition which is poorly understood. Symptoms of IBS include abdominal pain, bloating, and bowel problems such as constipation or diarrhoea.

The new research shows that people with IBS have higher than usual levels of nerve fibres expressing the pain receptor TRPV1, responsible for causing a burning sensation when people eat chilli peppers. The study's authors, from Imperial College London, hope that doctors could treat the pain that people with IBS experience by targeting and blocking this receptor. People with severe pain from IBS are currently treated with opiates, which can have serious side-effects. Painkillers such as paracetamol and ibuprofen tend to offer little relief. New painkillers to target TRPV1 are currently being developed by pharmaceutical companies and the new findings suggest that such drugs could tackle some of the symptoms of IBS.

The researchers believe their findings may explain why some people's IBS symptoms worsen after eating spicy food. They also suggest that the presence of more nerve fibres expressing the TRPV1 pain receptors might mean that people with IBS are more susceptible to pain.

Professor Subrata Ghosh, one of the authors of the study from the Division of Medicine at Imperial College London, said: "IBS can seriously affect people's quality of life and our new study could explain some of its symptoms. At the moment patients don't have a lot of options for managing their condition and the treatments we can offer can give disappointing results. We hope that our findings will lead to better treatments to help people with IBS."

Radiation Protection

X-ray aprons are the most important part of protecting yourself from radiation in the medical industry. Many professional are not educated on the dangers of exposing yourself to radiation for long periods of time. There are several solutions to this problem. It is imperative that medical professionals posses the knowledge about the dangers of radiation exposure. They must also be aware of the ways to protect themselves and their patients from x-ray exposure.
X-ray aprons are typically referred to sometimes as lead aprons. This is because traditionally x-ray protection aprons were made of lead because of its density and ability to attenuate radiation. However lead aprons also had some very serious downsides. The first issue is the main protecting agent is lead. Lead is not environmentally friendly, which creates a problem when it comes to disposing of the lead apron after several years of use. The other danger with traditional lead aprons is that they are not ergonomically designed. The lead is often very heavy and when a surgeon is wearing it during long procedures it can be very strenuous on their bodies, especially their backs.
Manufacturers have come a long way now from the old traditional lead aprons, and they have began testing other materials other than lead that can protect against radiation but are lighter in weight. New metal alloys have been developed by companies like Lite-Tech. Their brand of x-ray aprons named Xenolite contains no lead in them. Instead of lead other lighter metals have been used to create an alloy that protects at the same equivalence as lead.
While they are not made of lead they protect equally as well and in the case of Xenolite their non lead aprons protect at a higher level than their ones that do contain lead. You can view the results in the Columbia Report that Columbia University performed. They tested attenuation rates of all the major manufacturer’s of xray aprons in the US and found that the Xenolite non lead aprons had the highest attenuation rating.
When picking out an x-ray apron for radiation protection the two main issues to address is protection equivalence and ergonomic suitability of the apron.

Prevention of Decubitus Ulcers

Decubitus Ulcers can pose a serious threat to a patients health. The best treatment for decubitus ulcers is to prevent them entirely by taking the necessary precautions. It is very important to take the necessary precautions to prevent ulcers during surgical procedures. A patient’s body is prone to decubitus ulcers while they are having surgical procedures performed. These pressure sores can form in as short as 30 minutes of laying idle in one position.

The sores form due to the tissue not being able to receive blood from the pressure that the patients’ weight puts on the skin. Since surgical procedures require a patient to be anesthetized for long periods of time pressure sores can easily develop.

Prevention of decubitus ulcers is the best remedy to the problem. Not preventing these during surgical procedures is unacceptable and can be seen as surgical neglect. These ulcers are a serious matter and can last for months after a surgical procedure is done. In order to prevent a medical malpractice lawsuit it is essential to equip your facility or practice with the appropriate equipment that will prevent these pressure ulcers from forming.

The most common prevention method is to use surgical gel table pads and gel positioning products. These products are favored because they can be disinfected and reused over and over, unlike foam products which must be discarded for infection control purposes.
